Assisted Living Levels of Care and Fees Explained

Many assisted living communities do not charge one flat price. They assess each resident and assign a level of care, and the monthly fee follows. This guide explains how that usually works. Terms differ by community and by state.

What is a level of care?

A level of care is a community's way of grouping the help a resident needs, such as reminders, bathing help, mobility help or medication management. Each community defines its own levels and names.

How the assessment works

Before move-in, staff usually assess the person, often with a nurse. The assessment is typically repeated when needs change. Ask how often it happens and who decides.

What can raise the fee

  • More help with bathing, dressing or moving around
  • Medication management
  • Incontinence care
  • Increased supervision
  • Annual rate increases, separate from care changes

Questions to ask

  • How many levels of care are there and what does each include?
  • What is the typical range of cost between levels?
  • How much notice do you give before a fee change?
  • At what point would my family member need to move out?
  • Can I see the assessment form and the contract?

Read the contract

The contract should explain fees, discharge rules and refund terms. See our guide on contract red flags and consider asking an elder law attorney to review it.

Questions

Why did the price go up after move-in?

Often because care needs increased or the community raised its rates. Ask how both are handled before signing.

Are levels of care the same everywhere?

No. Each community sets its own, and state rules vary.
